How to file a notice

1. Your physical or electronic signature (or your authorized agent's).
2. Identification of the copyrighted work you claim was infringed.
3. Identification of the material to be removed, with enough detail to find it (the URL).
4. Your name, address, phone number, and email.
5. A statement that you have a good-faith belief the use isn't authorized by the owner, its agent, or the law.
6. A statement, under penalty of perjury, that the information is accurate and you're authorized to act for the owner.

Counter-notices

If your content was removed and you believe it was a mistake, reply to the same address with a counter-notice including your signature, identification of the removed material, and a statement under penalty of perjury that the removal was a mistake.
